PLEASE DO NOT EMAIL YOUR FORMS TO US, AS ORIGINAL SIGNATURES ARE REQUIRED!!! PHYSICALS MUST HAVE THE "INK" SIGNATURE AND FACSIMILES/PHOTOCOPIES CAN NOT BE ACCEPTED. YOU MUST MAIL OR DELIVER ALL OF YOUR FORMS TO BE VALID!!
THE REPORT CARD THAT YOU NEED TO SEND IN WILL BE THE YEAR END REPORT CARD THAT YOU RECEIVE IN JUNE!
If you need to verify or change your registration, you must contact our Registrar through our NRAA email. Please do not contact your requested coach!
Your registration is not complete and you will not be assigned to a team until full payment and your forms have been received (excluding the report card, which is due by July 15th). If your physical is due later in the year, please include a note to let us know and we will not hold up your team assignment. If you sent a note with your paperwork, you do not need to contact us to follow up.
You will receive an email from our Registrar upon processing of your paperwork to let you know that it has been received and what, if anything, still needs to be turned in. Please allow up to one week to receive this notification before emailing us to verify your child's status.

REGISTRATION FEES
Please visit the football or cheerleading information pages to determine in which level your child will be eligible to participate.
CHEERLEADING:
$85 (Flag, Tiny Mite and Mitey Mite levels) and $165 (Jr. Pee Wee, Pee Wee, Jr. Midget and Midget)
FOOTBALL:
$85 (flag football only) and $165 (all tackle teams including: Tiny Mite, Mitey Mite, Jr. Pee Wee, Pee Wee, Jr. Midget and Midget)
FAMILY DISCOUNTS
Discounts are automatically applied for children of the same family who are registered in the same online session! If you need to register in different sessions, you still qualify for the discount; click here for more information.
1st child pays full registration cost at most expensive level; 2nd child receives a $20 discount; 3rd child receives a $30 discount; and the 4th child is FREE!
REFUND POLICY
We understand that it may be necessary for participants to withdraw from the program for unforeseen reasons. Full refunds, less a $15 administrative fee, will be made available for any participant provided:
1. The request is made in writing on the required form prior to the participant's second game of the season. After the participant's second game, refunds will not be available.
2. ALL equipment has been returned to the equipment manager and the equipment manager has signed the form indicating date of return.
3. The written request is sent to the Registrar at the email indicated on the form.
4. Please allow 3-4 weeks for processing of a refund.

WHAT PAPERWORK IS REQUIRED FOR REGISTRATION?
Each player must provide the following information in order to have a spot on a team. These items will be made available during your online registration session, except the medical form which you may download now.

Copy of player's birth certificate.

Completed Pop Warner Participation Contract.

Completed physical form. It must be the physical form provided on our website. You may not submit a physical form from the doctor, charts or immunizations records. DOWNLOAD YOUR 2010 MEDICAL FORM. THIS FORM MUST BE DATED AFTER 1/1/10 AND NO OTHER FORM WILL BE ACCEPTED!

WHAT IS THE NEW ONLINE REGISTRATION PROCESS?
Returning participants will be able to register on April 1st and new participants may register beginning April 17th, both at 8am. On April 17th, we will hold our annual on-site registration event at WMMS from 8am until noon. While participants may register in person at this event, we highly recommend that you register online and use this time to turn in paperwork and/or remit payment, as it will greatly help our volunteer administrators save hundreds of hours by not having to enter your data by hand! At our registration event, you will also have the opportunity to meet our coaches and have your questions answered.
STEP-BY-STEP INSTRUCTIONS
1. Go online through our website to fill in all of the blanks and make payment by credit card or indicate that you will mail a check. At the end of the session, you will receive a receipt that contains links to print forms that require your original signature. Include a copy of your valid medical insurance card and a copy of your child's birth certificate. Mail or bring them in to our registration event. Please note that your registration is not complete and your child will not be assigned to a team until forms are received!
2. Take page two of the medical form to your physician to complete. This form MUST be dated after January 1st and MUST be an original. Send this in with all forms from #1 OR send a note as to when your child's physical is due if it has not yet occured. We will need to have a physical on hand prior to allowing your child to participate in any practice.
3. After your child completes their school year, mail, fax or email the complete report card by July 15th. The report card is not acceptable unless it:
-INCLUDES ALL FOUR QUARTERS, and
-SHOWS PROOF OF PROMOTION: If it does not, the school can verify this by: 1) an email to our Scholastics Coordinator that will be printed and attached; 2) the school official can write directly on the report card; or 3) the school can fax a note directly.
